Ülseratif kolit hastalarında değişen ne? Yeni tanı almış 99 vakanın retrospektif değerlendirilmesi

Çalışmaya Ait Türkçe Öz

Amaç: Ülseratif kolit (ÜK) insidansı, potansiyel
çevresel risk etkenlerine maruziyetteki artış ile yükselme eğilimindedir. Bu
çalışmada, insidanstaki bu artışın klinik başvuruda farklılıklar oluşturup
oluşturmadığının araştırılması amaçlanmıştır.

Yöntem: Çalışma
retrospektif, hastane bazlı çalışmadır. Ağustos 2017 ile Eylül 2019 tarihleri
arasında başvuran ve yeni tanı almış, yetişkin ÜK’li hastalar çalışmaya dahil
edilmiştir.

Bulgular: Çalışmaya 99 ÜK’li olgu alındı; erkek
kadın oranı 1,54 olarak bulundu. Cinsiyete göre yaş ortalamalarına
bakıldığında; erkeklerde ortalama yaş 45,67 (SD±17,02) iken kadınlarda 37,67
(SD±12,47) bulundu. Aralarındaki fark istatiksel olarak anlamlıydı (p=0,01;
p<0,05). 30-39 yaş grubu hastalığın en sık görüldüğü yaş aralığı olarak
saptandı (%30,3). Cinsiyete göre tutulum sıklığı erkeklerde 3 bölge için hemen
hemen aynıyken (%33,9 pankolit, %33,9 sol kolit, %32,2 distal kolit) kadınlarda
sol taraf tutulumunun belirgin düşük olduğunu görüldü (%42,4 pankolit, %18,2
sol kolit, %39,4 distal kolit). Cinsiyetler arasındaki bu fark istatiksel
olarak anlamsızdı. Erkeklerde başvurudaki endoskopik aktivite indeksi kadınlara
göre, istatiksel olarak anlamlı, belirgin yüksekti (median 3 ve 2 sırasıyla;
min 0 max 3; p=0,01, p<0,05).










Sonuç: Çalışmamız bu hastalığın hem daha ileri
yaşta hem de daha şiddetli bir şekilde erkeklerde daha sık görüldüğünü ortaya koydu.
Buna rağmen kesin yargıya varılabilmesi için geniş çaplı, toplum kökenli
çalışmalara ihtiyaç vardır.

What's changing in ulcerative colitis patients? Retrospective evaluation of 99 newly diagnosed cases

Objective: The incidence
of ulcerative colitis (UC) tends to increase with increment exposure to
potential environmental risk factors. In this study, it was aimed to
investigate whether this increase in incidence causes differences in clinical
presentation.

Methods: This is a retrospective, hospital-based
study. Between August 2017 and September 2019, newly diagnosed adult UC
patients were included in the study.

Results: 99 patients with UC were included in the
study; male to female ratio was found to be 1.54. When the average age is
examined by gender; The mean age was 45.67 (SD ± 17.02) in males and 37.67 (SD±12.47) in females. The difference was statistically significant (p=0.01; p
<0.05). 30-39 age group was found to be the most common age range (30.3%).
While the frequency of involvement by gender was almost the same for 3 regions
in men (33.9% pancolitis, 33.9% left colitis, 32.2% distal colitis), it was
found that left side involvement was significantly lower in women (42.4%
pancolitis, 18.2% left colitis, 39.4% distal colitis). This difference between
the sexes was statistically insignificant. At the time of admission, the
endoscopic activity index was notably higher in men than in women. This
difference was statistically significant (median 3 and 2, respectively; min 0
max 3; p=0.01, p <0.05).










Conclusion: Our study
revealed that this disease is more common in men, both at an older age and more
severely. Nevertheless, large-scale, community-based studies are needed to
reach a final judgement.
