Abstract
In this report, we aimed to present a case of suture reaction, which is a rare complication that developed after thyroidectomy surgery and presented with a fistula on the neck, which could not be achieved by many surgeries. A 50-year-old female patient was admitted to the General Surgery Clinic of the Istanbul Training and Research Hospital with the complaint of fistula at the incision site of goiter, which has been going on for 14 years. She had bilateral thyroidectomy surgery 14 years ago in a different hospital than ours and her pathology was found to be benign. 4 months after the operation, swelling developed around the neck and the leakage from the neck did not cease despite 3 surgical interventions. In the examination of the patient, a fistula mouth was detected on the left and 5 cm below the old incision scar on the anterior neck. On the neck tomography taken to evaluate the patient, a foreign body granuloma was detected deep on the left. Methylene blue was given from the fistula tract of the patient who was taken into surgery. The fistula in the neck was followed to the deep planes with an incision covering the mouth of the fistula, and the bottom part was ligated and cut. Fistula excised all around. When the section of the excised piece was observed, a sinus with sutures due to the operation was observed and pathological examination confirmed this situation. The cause of the pathology should be investigated before the surgical intervention in every patient presenting with swelling and abscess in the neck after thyroid surgery. Surgical intervention is absolutely necessary in cases with granuloma sinus. In cases with fistula, it was concluded that the total removal of the formed
granuloma sinus by following the fistula tract would permanently eliminate the complaints.

Abstract
Bu yazıda tiroidektomi ameliyatı sonrası gelişen ender bir komplikasyon olan ve birçok kez cerrahi yapılarak sonuç alınamayan boyunda fistül ile gelen sütür reaksiyonunu olgusunu sunmayı amaçladık. 50 Yaşında kadın hasta İstanbul Eğitim Araştırma Hastanesi Genel Cerrahi Kliniğine 14 yıldır süre gelen guatr insizyon yerinde fistül şikâyeti ile baş vurdu.14 yıl önce bir dış hastanede bilateral tiroidektomi ameliyatı olmuş ve patolojisi benign gelmişti. Operasyondan 4 ay sonra boynunda şişlik gelişmiş ve 3 kez cerrahi girişim uygulanmasına rağmen boyundaki akıntı kesilmemişti. Hastanın yapılan muayenesinde boyun ön yüzde eski insizyon izinin 5 cm altında ve solda fistül ağzı tespit edildi. Hastayı değerlendirmek amaçlı çekilen boyun tomografisinde solda derinde yabancı cisim granülomunu saptandı. Ameliyata alınan hastanın fistül traktından metilen mavisi verildi. Fistül ağzını içine alan insizyonla boyunda fistül derin planlara kadar takip edildi ve dip kısmı bağlanarak kesildi. Fistül çepeçevre eksize edildi. Çıkarılan piyes kesiti izlendiğinde ameliyata bağlı sütürlerin bulunduğu sinüs gözlendi ve patolojik tetkiki de bu durumu teyit etti. Tiroit ameliyatı sonrası boynunda şişlik apse ile gelen her hastada cerrahi müdahale öncesi patolojiyi oluşturan neden araştırılmalıdır. Granülom sinüsü saptanan olgularda mutlak olarak cerrahi girişim gerekir.
Fistül gelişen olgularda ise fistül traktını takip edilerek oluşmuş granülom sinüsün total olarak çıkarılmasının, şikayetleri kalıcı olarak ortadan kaldıracağı kanısına varıldı.
