The Role of Hematological Parameters in the Etiology of Missed Abortion

Abstract

Aim: To compare mean platelet volume (MPV) values and platelet counts of normal pregnancies and patients with missed abortions to determine whether hematological parameters such as MPV and platelet counts (PLT) can predict unsuccessful pregnancy particularly missed abortion.
Material and Methods: This retrospective case-control study investigated the hematological parameters of patients diagnosed with missed abortion and the control group. Among 476 cases, 238 participants with missed abortion were included in the study group and 238 healthy pregnant women were included in the control group. The mean age of the missed abortion group was 33.50 and the mean age of the control group was 32.
Result: Age showed a significant difference between the two groups (p = 0.01). Gestational age was higher in the missed abortion group than that in the control group (p < 0.001). Significant differences in gravida (p < 0.001) and parity (p = 0.002) were observed. Smoking was higher in the missed abortion group than that in the control group (p < 0.001). RDW-SD (p < 0.001) and hematocrit (p = 0.032) showed significant differences between groups. There was no significant correlation between MPV (p = 0.155; OR = 1.097; CI = 0.966_1.247), PDW (p = 0.695; OR = 0.991; CI = 0.947_1.037), PCT (P = 0.468) and missed abortion.
Conclusion: Hematological parameters such as MPV may not be used as a screening or diagnostic test for unsuccessful pregnancies particularly missed abortion as shown in the current study.

Hematolojik Parametrelerin Geçikmiş Düşük Etiyolojisindeki Rolü

Abstract

Amaç:Çalışmamızda ortalama trombosit hacmi (MPV) ve trombosit sayısı (PLT) gibi hematolojik parametrelerin başarısız gebelikleri, özellikle gecikmiş düşüğü tahmin etmede kullanılıp kullanılmayacağını belirlemeyi amaçladık.
Gereçler ve Yöntem: Bu retrospektif vaka-kontrol çalışmasında, gecikmiş düşük tanısı almış hastaların ve kontrol grubunun hematolojik parametreleri incelendi. Gebeliği normal seyreden gebeler ile gecikmiş düşükle sonuçlanan gebelerin MPV değerleri karşılaştırıldı. Çalışma grubuna dahil edilen gecikmiş düşük tanılı 238 hasta ve kontrol grubuna dahil edilen 238 sağlıklı gebe olmak üzere toplam 476 kişi çalışmaya dahil edildi. Gecikmiş düşük tanılı grupta ortalama yaş 33.50 iken, kontrol grubunda ortalama yaş 32 yıl idi.
Bulgular: İki grup arasında yaş açısından istatistiksel anlamlı fark mevcuttu (p=0.01). Gecikmiş düşük grubunda gebelik yaşı, kontrol grubuna göre istatistiksel anlamlı daha yüksekti (p<0.001). Gebelik sayısı (p<0.001) ve canlı doğum sayısı (p=0.002) açısından istatistiksel anlamlı farklılık saptandı. Gecikmiş düşük grubunda sigara içen hasta sayısı, kontrol grubuna göre daha yüksekti (p<0.001). RDW-SD (p<0.001) ve hematokrit (p=0.032) değerleri gruplar arasında anlamlı farklılık gösterdi. MPV (p = 0.155; OR = 1.097; CI = 0.966_1.247), PDW (p = 0.695; OR = 0.991; CI = 0.947_1.037), PCT (P = 0.468) değerleri ve gecikmiş düşük arasında anlamlı bir korelasyon bulunmadı.
Sonuç: Çalışmamızda gösterildiği üzere, MPV gibi hematolojik parametrelerin gecikmiş düşük için tarama veya tanı testi olarak kullanılması uygun bulunmamıştır.
