Investigation of Borreliosis and Leptospirosis frequency in patients with chronic renal failure and renal transplantation

Abstract

Bu çalışmada kronik böbrek yetmezliği ile böbrek nakli yapılan hastalarda B. burgdorferi ve Leptospira prevalansının araştırılması ve etyolojilerinde bu etkenlerin yerinin belirlenmesi amaçlandı.
Gereç Yöntem
Çalışmaya nefroloji polikliğinde takipli 193 gönüllüden oluşan hasta grubu ve bilinen kronik rahatsızlığı olmayan 91 gönüllüden oluşan kontrol grubu olmak üzere toplam 284 kişi dahil edildi. Hasta grubu; kronik böbrek yetmezliği ile takip edilen 174 hasta (% 90.2) ve böbrek nakli nedeniyle takipli 19 (%9.8) hastadan oluşmaktaydı. 174 KBY hastasının etyolojisinde 79’unda (%45.4) hipertansiyon, 32’sinde (%18.3) diyabet, 5’inde (%2.9) glomerulonefrit, 46’sının (%26.5) nedeni bilinmiyordu. 284 serum örneğine Treponema pallidum tarama testi olan RPR testi ile Leptospira IgG ve B. burgdorferi (Viro-Immun Labor-Diagnostika IgG, Almanya) IgG varlığının araştırılması için ELISA yöntemi ile çalışıldı.
Bulgular
Çalışmaya alınan hasta ve kontrol grubunda T. pallidum için yapılan tarama testi olan RPR testinde bütün örnekler negatif bulundu. B. burgdorferi IgG hasta grubunda 4 (% 2.1) hastada pozitif, kontrol grubunda 2 hastada pozitif (% 2.2) olarak bulundu. Leptospira IgG hasta grubunda 27 (%13.9) hastada pozitif kontrol grubunda 3 hastada (%3.3) pozitif bulundu. Leptospira IgG pozitifliği yönünden çalışma ve kontrol grubunda arasında anlamlı fark vardı (p=0.006) . Etyolojik tanı gruplarına göre Leptospira ve Borrelia IgG seropozitifliği açısından gruplar arasında anlamlı fark yoktu (p>0.05).
Tartışma
B. burgdorferi ve Leptospira enfeksiyonlarında, hastalar erken tanı ve tedavi alamamaktadırlar. Bu durumda, özellikle Leptospira olgularında kronik böbrek yetmezliğine gidiş olabileceği unutulmamalıdır. Bu nedenle böbrek tutulumu olan hasta grubunda özellikle kırsal kesimde yaşama, hayvancılık ve ormancılıkla uğraşma gibi risk faktörü olan hastalarda B. burgdorferi ve Leptospira enfeksiyonları akılda bulundurulmalı ve serolojik yöntemler ile en azından tarama testi yapılmalıdır.
