Efor Testi Anormal Saptanan Hastalar ile Klinik, Ekokardiyografik, Biyokimyasal ve Hematolojik Parametrelerin ilişkisi

Abstract

Amaç: Anormal egzersiz stres testi olan hastalar ile normal egzersiz stres testi olan hastaların klinik, ekokardiyografik ve biyokimyasal parametrelerini karşılaştırmak Yöntem: Çalışmaya koroner arter hastalığından şüphelenilen 65 hasta dahil edildi. Tüm hastalara transtorasik ekokardiyografi uygulandı. Biyokimyasal ve hematolojik parametreler çalışıldı. Tüm hastaların Framingham, PROCAM, SCORE ve TKD risk skorları hesaplandı ve Bruce protokolüne göre egzersiz stres testi uygulandı. Hastalar normal ve anormal efor testi olmak üzere iki gruba ayrıldı. İki grup arasında parametreler student t test ile değerlendirildi. Bulgular: Çalışmada yer alan hastaların ortalama yaşı 52.9 (%53 erkek) idi. Hastaların %7.6 diyabet, %40.9 hipertansiyon ve %22.7 ‘si sigara kullanıcısı idi. Hesaplanan risk skorları Framingham, PROCAM, SCORE ve TKD (Türk Kardiyoloji Derneği risk skoru) iki grup arasında benzerdi. Bununla beraber anormal egzersiz stres test ile epikardiyal yağ dokusu ve aortik elastik parametreleri (aortik strain ve aortik distensibilite) arasında bir ilişki tespit edilmedi. Anormal egzersiz stres testi olan grupta normal gruba göre trombosit sayısı ve trombosit dağılım genişliği arasında anlamlı farklılık saptandı. ( 298.000 vs 253.000 p:0.035, 16.02 vs 14.8 p: 0.027, sırasıyla). Sonuç: Çalışmamıza göre trombosit sayısı ve trombosit dağılım genişliğinin anormal egzersiz stres testi olanlarda yüksek olması, koroner arter hastalığının ortaya çıkmasında trombositlerin anahtar rol oynadığının göstergesi olabilir.
