The Philosophy and Importance of Choosing Wisely in the post-COVID-19 Pandemic Era

Abstract

Young physicians inherited the principle of 'First Do No Harm' from Hippocrates, considered the father of medicine. It is necessary to expand this principle's scope, which is also the importance and priority today, as "First Do No Harm and Be Helpful," and choose the practice that provides benefits while emphasizing safety in patient care.
In this context, the “Choosing Wisely philosophy” ; aims to prevent excessive and unnecessary exams, diagnosis and treatment processes by making rational choices to provide the minimum harm as well as the maximum benefit to the patient. The COVID-19 pandemic, which changed the socio-cultural and economic balances worldwide, has enabled countries to reconsider and question their health systems and reminded us once again how important it is to use existing resources rationally.
It has become even more important to prevent excessive and unnecessary use of available resources by making rational choices in the post-COVID-19 pandemic World. This article will focus on the rational management of resources in the COVID-19 and post-COVID-19 pandemic, the "Choosing Wisely" campaign, its place, and its importance in the COVID-19 pandemic and post-COVID-19 pandemic World.

COVID-19 Pandemisi sonrası Choosing Wisely Felsefesi ve Önemi

Abstract

Tıbbın babası olarak kabul edilen Hipokrat’tan genç hekimlere ‘Önce Zarar Verme’ ilkesi miras kalmıştı. Günümüzde önemi ve önceliği devam eden bu ilkenin kapsamını ‘Önce Zarar Verme ve Faydalı Ol’ olarak genişleterek, bir yandan hasta bakımında güvenliği öne çıkarırken diğer yandan da fayda sağlayan uygulamaların seçilmesi gerekli görülmektedir. Bu bağlamda Choosing Wisely felsefesi; akılcı seçimler yapılarak aşırı ve gereksiz tetkik, tanı ve tedavi süreçlerinin engellenmesini ve bu şekilde hastaya en fazla faydanın yanında en az zararın verilmesini sağlamayı amaçlamaktadır. Tüm dünyada sosyokültürel ve ekonomik yönden dengeleri değiştiren COVID-19 pandemisi ülkelerin sağlık sistemlerini yeniden gözden geçirmelerini ve sorgulamalarını sağlamış, var olan kaynakların akılcı kullanılmasının ne kadar önemli olduğunu bir kez daha bizlere hatırlatmıştır. COVID-19 pandemisi sonrası dünya gündeminde iklim krizi başta olmak üzere her gün yeni krizler ortaya çıkmakta ve tüm bu sorunların yönetiminde akılcı seçimler yapılarak kaynakların etkin kullanımı öne çıkmaktadır. Bu yazıda COVID-19 pandemisi sonrası yeni dünyada kaynakların akılcı yönetimi ana başlığı ile “Choosing Wisely” felsefesinin özellikle sağlık sistemimizdeki önemi üzerinde durulacaktır.
