Abstract
Diastematomiyeli, spinal kordun yarılmasıyla ortaya
çıkan, fibröz veya kemik bir septum ile ayrılabilen iki hemikord oluşumuna
neden olan, nadir bir okült spinal disrafizmdir. Diastematomiyeli izole
olabileceği gibi, miyelomeningosel, meningosel, spinal lipom, dermal sinüs,
vertebral cisimlerin diğer segmental anomalileri veya viseral malformasyonlar
gibi diğer disrafizmlerle ilişkili olabilir. Bu malformasyonun izole formu iyi
prognoza sahiptir. Bu nedenle, diastematomiyeli saptandığında doğru ebeveyn
danışmanlığı sağlamak için, diğer anomalileri dışlamak adına prenatal
değerlendirme dikkatle yapılmalıdır. Dikkatli sonografik inceleme ile
diastematomiyelinin prenatal tanısı mümkündür. Spinal kanalda ekstra ekojenik
posterior odak saptanması, bu durum için oldukça spesifik bir prenatal
işarettir. MRG, diğer ilişkili malformasyonları dışlamak için faydalıdır. İzole
diastematomiyeli, çok az veya ilişkisiz defisitle ortaya çıkabilir. Burada
rutin ikinci trimester sonografisinde tespit edilen izole bir diastematomiyeli
olgusu sunulmaktadır.
