Kronik B ve C Hepatitli Hastalarda Hepatik Vendeki Doppler Ultrasonografi Bulguları ile Histopatolojik Parametrelerin Karşılaştırılması

Abstract

Bu çalışmada asemptomatik kronik B ve C hepatitli hastalarda hepatik vendeki Doppler ultrasonografi bulgulan ile histopatolojik parametreler karşılaştırarak hastalığın progresyonunun gösterilmesin¬de Doppler Ultrasonografınin başarısı araştırıldı.
Prospektif olarak 43 kronik viral hepatitli ve 25 sağlıklı olguda hepatik vendeki akım formu değerlendirildi. Bulgular sağlıklı ve hepatit gruplan inflamasyon ve fıbrozis açısından derecelere ayrılarak karşılaştırıldı. Hastalar inflamasyon derecesine göre dört gruba (mi¬nimal, hafif, orta ve şiddetli); fıbrozis evrelerine üç gruba (minimal, orta ve şiddetli) ayrıldı.
Hasta grupta hepatik vende trifazik olmayan akım formu sıklı¬ğında anlamlı artış kaydedilmiş, ancak grade ve evre alt gruplarının birbirleri ile karşılaştırılmasında anlamlı farklılık gösterilememiştir.
Hepatik venin Doppler US ile değerlendirilmesi kronik viral hepatitli hastalan saptamada başarılı olmakla birlikte hastalığın progresyonunun gösterilmesinde sınırlı faydaya sahiptir.

Abstract
Wc aimed to compare histopathologic parameters with portal vein and hepatic artery Doppler ultrasonographic (USG) findings to assess progression of disease without need for a biopsy in the asymptomatic hepatitis B and C patients.
This prospectively designed study included 43 chronic hepatitis patients and 25 healthy subjects. Patient group is divided into four subgroups (minimal, mild, modarate and severe) regard to inflammation and three subgroups regarding to fibrosis (mild, moderate and severe). Obtained data from subjects compared between healthy subjects and patient group as well as between subgroups.
Increase in the frequency of non-triphasic flow pattern of hepatic vein were found among patients with chronic hepatitis comparing with the healthy group. However no difference is found between inflammation and fibrosis subgroups about examined indices.
We found that Doppler USG examination of hepatic vein is a sensitive method for identifiying of the chronic viral hepatitis but it has limited value for demostrating disease progression.
