Servikal Disk Herniasyonu Olan Hastalarda Yaşam Kalitesi, Özür, Fonksiyonel Durumun Radikülopati ile İlişkisi

Öz

Amaç: Servikal Disk herniasyonlu (SDH) radikülopatisi
olan ve olmayan hastalarda yaşam kalitesi, özür, fonksiyonel durumlarını
belirlemek ve radikülopati varlığının bu parametreler ile ilişkisini
belirlemektir.



Gereç-Yöntem: SDH’lı, 25- 65 yaş aralığında, en az 8 haftadır
ağrısı olan ve motor kaybı olmayan 37 radikülopatili ve 14 radikülopatisi
olmayan toplam 51 hasta alınmıştır. Hastaların özür seviyesi boyun özür skalası
(BÖS), üst ekstremite ile ilişkili özür seviyesini belirlemek için kol-omuz-el
sorunları anketi (DASH), yaşam kalitesini belirlemek için Nottingham Sağlık
Profili (NSP), kavrama kuvveti Jamar el dinamometresi ile
değerlendirilmiştir.  Gruplar arası fark
Mann- Whitney U testi ve parametreler arası korelasyon Spearman testi ile
belirlenmiştir.



Sonuç: Radikülopati varlığında ait yaşam kalitesi,
fonksiyon parametrelerinde değerler daha yüksek ve istatistiksel olarak farklı
bulunmuştur (p<0,05). Kavrama kuvveti ve BÖS değerleri gruplar arasında
farklı bulunmamıştır(p>0,05). Radikülopatisi olan hastalarda  boyun özür ve fonksiyon parametrelerinin
yaşam kalitesi, kavrama kuvveti ve birbirleriyle olan ilişkisi incelendiğinde
DASH ile yaşam kalitesi toplam puanı arasında pozitif yönde iyi derecede
korelasyon, fiziksel aktivite, sosyal izolasyon ve ağrı parametreleri arasında
pozitif yönde orta dereceli korelasyon bulunmuştur (p<0,05).



Tartışma: Servikal radikülopati ile yapılan bir çok
çalışmada birincil sonuç ölçümü olarak BÖS kullanılmaktadır, fakat üst
ekstremite  fonksiyonlarının
radikülopatide önemli ve değerlendirilmesi gereken anket olduğu
unutulmamalıdır.

Quality of life, disability, functional status and relationship with radiculopathy in patients with cervical disc herniation

Öz

Purpose: To determine the quality of life, disability, and functional status of patients with cervical disc
herniation (CDH) that accompanied with and without radiculopathy and to determine the relationship
between parameters and radiculopathy. Material and Methods: Fifty-one patients with CDH between 25
and 65 years, having neck pain at least 8 weeks and without motor loss were participated to the study.
Participants allocated into two groups as having radiculopathy (n=37) and without radiculopathy (n=14).
Cervical compression and distraction test for radiculopathy existence, Neck Disability Index (NDI) for
disability level, Disabilities of the Arm Shoulder and Hand Questionnaire (DASH) for upper extremity
related disability, Nottingham Health Profile (NHP) for quality of life with and Jamar hand dynamometer
for grip strength were used. Results: Quality of life and upper extremity related disability were higher
and different in patients with radiculopathy (p<0.05). There was no difference between groups in terms of
grip strength and NDI (p>0.05). There was positive and good correlation between DASH and NHP;
positive and moderate correlation between physical activity, social isolation, pain sub-parameter of NHP
and DASH in patients with radiculopathy (p<0.05). Conclusion: Patients who had radiculopathy
additionally to CDH, quality of life and functional status parameters of upper extremity were found to be
more affected than patients without radiculopathy. In patients with radiculopathy, It has been observed
that disability might not cause only by neck pain but also by its insufficiency in upper extremity related
functional status.
