Osteomeatal Kompleksteki Anatomik Varyasyonların Kronik Maksiller Sinüzit ile İlişkisi

Öz

Amaç: Maksiller sinüsteki mukoza (Schneiderian membran) kalınlaşmasının ≥2 mm olduğu durumun patolojik (Kronik Maksiller Sinüzit [KMS]) olarak kabul edilmesi gerektiği önerilmiştir. KMS’nin etiyopatogenezinde, tekrarlamasında ve akut/kronik inflamasyonun devam etmesinde paranazal sinüslerin ve Osteomeatal Kompleks (OMK) anatomisindeki varyasyonların etkisi olduğu iddia edilmektedir. Bu çalışmanın amacı, Konik Işınlı Bilgisayarlı Tomografi (KIBT) görüntüleri üzerinden radyolojik KMS varlığı/yokluğu ile OMK’deki anatomik varyasyonlar arasındaki ilişkiyi değerlendirmekti.
Yöntem: Çalışmada maksiller sinüs ve OMK’in birlikte gözlenebildiği 18 yaş ve üstü hastaların tam yüz KIBT görüntüleri incelendi. Maksiller sinüsteki mukozada 2 mm ve daha fazla kalınlaşma KMS'nin radyolojik bulgusu olarak kabul edilerek çalışma grubunda 50, kontrol grubunda 50 olmak üzere toplam 100 hasta çalışmaya dahil edildi. Araştırılan anatomik varyasyonlar; Nazal Septum Deviasyonu (NSD), Konka Bülloza (KB), Haller Hücresi (HH) ve Maksiller Aksesuar Ostium (MAO) idi. İstatistiksel analiz için Ki-kare (χ2) testi kullanıldı ve p<0,05 istatistiksel olarak anlamlı kabul edildi.
Bulgular: 18-82 yaş aralığındaki hastaların yaş ortalaması 47,57 idi. Çalışma grubunda, en az bir varyasyon gözlenme sıklığı kontrol grubuna göre istatistiksel olarak anlamlı seviyede daha yüksekti (%96>%76) (p<0,05). Çalışma grubundaki sıklığı kontrol grubundaki sıklığına göre istatistiksel olarak anlamlı farklılık gösteren tek varyasyon NSD idi (%64>%38) (p<0,05).
Sonuç: OMK’deki anatomik varyasyonlar maksiller sinüsü KMS için yatkın hale getirmekle birlikte KMS’nin mutlaka bir varyasyonun varlığı ile ilişkili olduğu anlamına gelmemektedir. Bu çalışmada KMS hastalarındaki en yaygın varyasyonlar NSD ve AMO idi. Ancak, sadece NSD’nin KMS’nin radyolojik bulgusu olarak kabul edilen 2 mm ve üstü mukozal kalınlaşma ile anlamlı bir ilişkisi bulundu.
